To submit a life claim, please provide the following:

 

  1. The completed Claimant Statement
    (one for EACH beneficiary)

  2. Each beneficiary's driver's license or state ID

  3. Clear copy of final death certificate with the cause of death

  4. If a portion of death benefits are assigned to a funeral home or funding company, a copy of the signed and notarized assignment and funeral home invoice

To submit an annuity claim, please provide the following:

 

  1. The completed:

a. Annuity Claimant Statement
    (one for EACH beneficiary)
b. IRS Form W-4P Withholding Certificate
    (one for EACH beneficiary)
c. Spousal Rollover Form *
d. 5 Year Hold Form *

  1. *Not eligible if beneficiary is the Estate or a Trust

  2. Each beneficiary's driver's license or state ID

  3. Clear copy of final death certificate with the cause of death for processing.
