Rhode Island agents: GU1338 filing procedures

The GU1338 is a special form of proof of auto insurance required by Rhode Island Department of Motor Vehicles for specific classes of risks.

This form must be on file with the DMV before they will register the following types of vehicles:

• Minor DOB (under the age of 18)
• Motor vehicle for hire
• Taxicab and limited public motor vehicles
• Auto body shop
• Dealer plates
• Bailee plates
• In-transit plates
• School buses
• Transporters
• Fuel delivery vehicles

Dealer plates and transporters are most frequently used by Phenix Mutual.

The agent must send in a copy of the dealer’s registration to ensure we have the correct name on the GU1338 form.

This form is effective until the policy cancels or is nonrenewed. It does not need to be issued on an annual basis. At that point, we would use the SR-26 form for Cancellation or Termination.
