Reminder: Applications changed March 1

Motorists Life updated its new business applications due to regulatory changes. Here are important notes about form ICC 10-677 (6-16) and ML-675FL (6-16). These applications are in all UWNB state app packets.

• We no longer accept version ICC10-677 (4-12); FL ML-675FL (4-12).
• You must use ICC 10-677 (6-16) and ML-675FL (6-16).
• Any apps using the old forms dated after Feb. 28, 2017, are being returned, and the agent is required to fill out a new application.
