New apps for 2017

Motorists Life has updated its new business applications due to regulatory changes. Here are important notes about form ICC 10-677 (6-16) and ML-675FL (6-16). These applications are in all state app packets for UWNB.

• We will no longer accept version ICC10-677 (4-12); FL ML-675FL (4-12) as of Feb. 28, 2017.
• Please use ICC 10-677 (6-16) and ML-675FL (6-16).
• Any apps dated after Feb. 28 will be returned and the agent will be required to fill out a new application.
