Report Injury

Policyholders should report workplace injuries to Encova within 24 hours of learning of the injury.

 

Benefits of Early Reporting

Early reporting of injuries helps everyone involved. It helps the policyholder keep claims costs and premiums down. It helps the injured employee by allowing the claims adjuster to make faster decisions, getting them the medical care they need in order to return to work. It helps Encova know how to react and who we need to involve in our 360° workers’ compensation approach. Finally, it helps the entire system by cutting down on potential problems associated with unmanaged claims.

Options for reporting a claim:

  1. Report electronically through Encova Edge. Contact your Encova representative for information about becoming an Encova Edge user.
  2. Call 844-362-6821, and select “policyholder” and option 1.
  3. Email the First Report of Injury to .
  4. Fax the First Report of Injury to 877-293-5513 or 304-941-1151.


Options for accessing the First Report of Injury form:

  1. Visit the specific state website.
  2. Click the Virtual Claims Kit link in Encova Edge, choose the appropriate carrier and state and locate the correct form.

Injured employees should seek the fastest emergency care available. In a non-emergency situation, the employer should help the injured employee locate a physician from our medical provider network or provide them with a preselected panel where the jurisdiction permits.
